2023-2024 Winter Sport Information

 

Below is information pertaining to the Winter 2023-2024 Sports Season. Please complete the following 3 steps. Please read the message in its entirety. 

 

1. ONLINE REGISTRATION

Athletic Department Winter Sport Online Registration through FormReLeaf will open on Monday, October 2, 2023 at 9:00am. The registration process includes Permission to Participate, Emergency Medical Information, Student Athlete Code of Conduct as well as the NJSIAA Concussion, Sports Related Eye Injuries, Opioid, Steroid, and the Sudden Cardiac Death Awareness Information Forms.  In addition, a 6 minute Opioid Video, Athletes vs Opioids must be viewed by prospective student athletes and their parents(s)/guardian(s) prior to registration.  A sign-off form stating the video was viewed will be included along with the other acknowledgements. Registration can be completed by clicking on the FormReLeaf link. Please contact Tech Support at 1-844-367-6735 if any assistance is needed. 

 

2. PHYSICALS

Winter Sport Physicals - Physicals are due to the HS Health Office by Friday, October 13, 2023.  All student-athletes must have an approved physical before they are permitted to begin any athletic season.  Sports physicals are good for 365 days from the date of the exam.  If a physical expires during a particular sport, the student-athlete may continue until that sport is completed but must get a new physical before their next sport.  Physical Form

Health History Update Questionnaire - If you submitted a physical form to the Nurse's Office that was completed more than 90 days prior to the first day of official NJSIAA practice, you must submit a completed Health History Update Questionnaire signed by the student’s parent or guardian to the Nurses Office.     

Physicals and Health History Update Forms can be dropped off at the High School and placed in the white mailbox located outside of the entrance doors to the High School Main Office on the left or mailed to:

Please make a copy of your physical or any other paperwork before submitting it.  

 

3. SPORTS ACTIVITY FEE

The $100 Activity Fee Payment along with the completed form must be made payable to the Bridgewater-Raritan Board of Education.

It is RECOMMENDED that the Activity Fee Form and $100.00 check be turned in at the Athletic Office or mailed to the Athletic Office at:

ONLINE - Anyone electing to pay the $100.00 Activity Fee online must do so through MySchoolBucks or through your Parent Portal on PowerSchool.

*Please note: You will receive an Invoice under your MySchoolBucks account after final rosters have been made. 

2023-2024 Winter Sports Activity Fees are due by Friday, December 1st. Any athlete who does not process their payment will not be allowed to participate in practice on or after the due date until payment is made.

 

 

2023-2024 Winter Sport Starting Dates & Google Classroom Codes

 

Please be sure athletes join the Google Classroom for their sport to get updated information from their coaches. 

Try-outs for all teams will be a minimum of three days and can be as long as seven days. All candidates should arrive 30 minutes prior to the starting time of all try-out dates and practices. After try-outs are complete, practices will be held daily and all players are expected to attend all practices scheduled by their coach. Candidates are only permitted to participate in try-outs for one team during a season. Signing up for and participating in try-outs for more than one team at a time is not permitted.
